Hospice Companion Provider

Care Heart Ally Hospice Companion Servic

The Hospice Companion Provider offers essential comfort and companionship, ensuring your loved one feels safe and supported. They help the individual experience a smoother transition by monitors their condition and communicates effectively, helping to ensure necessary support is in place. Hospice companions assist family members in making sure their loved one is comfortable, collaborating with hospice RNs, LPNs, and CNAs while respecting each patient’s needs. The companion gives the family peace of mind and provide respite for primary caregivers when needed, while offering compassionate companionship and helping the individual stay safe. 

 

Duties:

Non-medical service will assist with the following:

 

  • Companionship​

  • Providing temporary relief to the primary caregiver by staying with the patient while they take a break

  • Observation, monitoring and reporting changes

  • Ensuring the patient comfort and managing anxiety

  • Ensuring patient's stays secure and safe from injuries or roaming around

  • Assistance with grooming and minimalizing bedsores

  •  Collaboration and maintaining open communication with the patient, family, and healthcare team to coordinate care and address concerns

  • Organizing personal space

  • Providing the Family with support, guidance and assistance

  • Providing resource for products and items to provide aid during the hospice care

Dementia & Alzheimer's Companion Provider

Care Hearts Ally Dementia & Alzheimer's Companion

Dementia and Alzheimer's Companion Providers offer compassionate companionship to loved ones, ensuring that seniors and their families feel secure knowing someone is always there to help. Companions serve as vigilant observers, ready to assist with daily needs and act swiftly in emergencies. By promoting increased social engagement and providing a sense of security, having a companion nearby can help prevent confusion and wandering, ultimately providing peace of mind for the family.​

 

Duties:

Non-medical service will assist with the following:

 

  • Compassionate Companionship​​

  • Help calling family and friends

  • Redirection wandering and confusion

  • Helping them stay clam from anxiety and sundowner's syndrome

  • Maintain safety and wellbeing from hazardous environment

  • Reassuring the individual is aware their family are well and available

  • Meal preparation 

  • Household chores

  • Light housekeeping

  • Laundry

  • Checking expiration of foods in the refrigerator, freezer cabinets and pantry

  • Emptying trash bins and taking the out trash

  • Non-medical Transportation

  • Escort on medical appointments, doctor's notes and correspondence

Companion Homemaker Service Provider

  • Conversation and Companionship​​

  • Help calling family and friends

  • Safety checks

  • ​Medication reminders (We Do Not Administer Medication)

  • Meal preparation 

  • Household chores

  • Light housekeeping

  • Laundry

  • Checking expiration of foods in the refrigerator, freezer cabinets and pantry

  • Emptying trash bins and taking the out trash

  • Non-medical Transportation

  • Escort on medical appointments, doctor's notes and correspondence

  • Escort on community outings

  • Daily documentation​

Respite Service Provider

Duties:

Non-medical service will assist with the following:

  • Help calling family and friends to reenforce social connections

Concierge Service Provider

The Personal Concierge Provider, is a helpful daily assistant. They tackle your to do list so you can spend your free time enjoying what you love. Concierges are great at taking care of daily tasks and your home management needs. 

 

Duties:

Non-medical service will assist with the following:

 

  • Meal and snack delivery​

  • Daily tasks

  • Running errands

  • Organizing refrigerator, freezer, cabinmates and pantry

  • Making bed

  • Emptying trash bins and taking the out trash

  • Minor home organization

  • Grocery shopping

  • Personal shopping

  • Pharmacy pick-up

  • Dry cleaning drop-off and pick-up

  • Pet meals, grooming and vet appointments

  • Plant care

  • Appointment setting

  • Waiting for repair service

  • Mailing, shipping and sorting
